Notice to 6th and 8th Grade Families-Hearing and Vision Screenings, Blood Pessure and BMI sceeenings- Opt out letters
We will be conducting Hearing and Vision Screenings for 6th and 8th graders on January 11, 2022 through a company called “Well Child”. This is a state mandated screening, and is no cost to you. If you would prefer your child not be screened, you must fill out the attached form and return it to your school’s front office no later than December 10, 2021.
The school nurse will also be conducting state mandated Blood Pressure and BMI screenings for 6th and 8th grade scholars starting January 17, 2021. Blood pressure sceenings involve a cuff placed around the scholar’s arm to measure blood pressure, and BMI involves measuring height and weight to calculate the BMI. These are quick, non-invasive screenings that measure important health metrics. If you would prefer your scholar to not be screened, you must ill out the attached opt out form and return it to your school’s front office no later than December 10, 2021.
